PSC investigators say several 'probable violations' played into the fatal accident that took the life of utilities worker Wendell Harrison, and burned 3 other crew members while they were replacing a section of corroded pipe at the intersection of 4th St Northwest and 4th Avenue Northwest on September 28th.

Among these violations, the report says 'prevention of accidental ignition', and a 'qualification program' were both lacking on the city's part.

Cairo officials met today (1/5) with the Pipeline Safety Director to discuss the findings. Following the meeting this morning, the commission will meet and vote to decide what, if any, fines and/or penalties the City of Cairo will face.

We'll keep you up-to-date as more information is released.
